Admissions
One of the cornerstones of
the Catholic faith is the core belief that all life in every
stage of life has immeasurable meaning and value. In living
out its mission as a Catholic facility, Immaculate Mary Home
stands as a witness to the unique dignity, destiny and worth
of each individual in our care.
The Admissions
Process
- Complete the required
forms and return them to us.
- If the applicant is
not currently in a hospital or an institution, please
have your physician fill out the physician's evaluation
form completely.
- The Admissions
Committee will review your completed application and
medical information.
- For your convenience a
Power of Attorney and irrevocable burial trust is
recommended prior to admission.
- You will be notified
once a decision has been reached.
- Should you have any
questions or require assistance with completing the
forms, please do not hesitate to contact us.
Other Admissions
Information
- Immaculate Mary Home's
basic rates include room, board, twenty-four hour nursing
care, religious services, social services, recreational
activities, housekeeping, maintenance and linen.
- Immaculate Mary Home
currently accepts referrals from hospitals and other
organizations across the Delaware Valley. Among other hospitals, our attending
physicians are affiliated with the following hospitals:
Nazareth Hospital, Frankford Health System, and Holy Redeemer Hospital.
- Insurances accepted by
Immaculate Mary Home include: Medicare, Keystone 65,
Personal Choice 65, and Aetna US Healthcare Golden
Choice.
